App crashes on clicking select image icon in Register Patient

(I am new to the community so please excuse me if i haven't followed any of the standards)

Actual Behaviour

App crashes on clicking select image icon in Register Patient

Expected Behaviour

App should not crash and should let the user select the desired image from gallery.

Steps to reproduce it

1) Log in and click Register Patient.

2) Click on the camera icon at the top.

3) App crashes (BUG).
