Context-sensitive tests for ImageDocumentHandler

A bunch of context sensitive tests are required to validate that reading, saving and deleting image attachments occur as expected and hedge the code base against any regressions.

This task should also encompass the refactoring of references to 'document(s)' into references to 'attachment(s)'.

For example ImageDocumentHandler should become ImageAttachmentHandler... etc.
