Add Integration Tests for Observation

See the Epic for details on what is to be tested. Tests should be for both the R3 and R4 version of the resource.

We should remove the update operation for Observation to stay in-line with OpenMRS's business rules.

Note that given the complexity of Observations, it would be good to demonstrate that they can be created for a variety of example Observations.
