Implement the Condition resource for OpenMRS 2.1 and lower

In OpenMRS 2.1, Conditions are represented not using a separate class but as observations that are coded using the CIEL code 1284 (and it's equivalents). We should implement the Condition resource to represent these Observations where we can.
