FHIR Client

We should provide some basic wrappers and utilities to get access to the correct IGenericClient instance and to translate OpenMRS objects into FHIR objects which can be sent using that interface. Ideally, this will reuse as much of the existing OpenMRS FHIR infrastructure as possible.
