htmlformentry: investigate using htmlwidgets module

Right now, the widgets in htmlformentry are done within the module itself. The htmlwidgets module aims to provide a rich and complete library for this which is well-tested, encapsulated, and reusable across multiple modules. We should consider integrating this in.
