Alphabetize the htmlform dropdown list

For many long lists, the dropdown should be capable of ordering the list (ie. alphabetical by family name). For the providers list in Rwanda, it should be alphabetized by "family name"? (says Benjamin Akimana) In Rwanda that would be last name for the Americans and Rwandan name for the Rwandans.)

Prefer to start with a default that works across different installations, but perhaps it could be customizable to familyName, familyName2, givenName, middleName.)
