<condition/> tag to support a formPath attribute

1. Background

Condition implements FormRecordable thereby allowing each of its saved entities to keep track of the exact form's position where they were recorded. HFE should leverage this mechanism.

2. Dev Notes

Such mechanism is currently implemented hacking addition detail to record the form field path. See for example here in ConditionElement.

3 Acceptance Criteria

This should be supported as a way to uniquely map a condition to the where it was recorded in the form:
