enrollinprogram tag needs to expand method for determining encounter date

Currently, if the enrollinprogram tag is used, and a form is opened retrospectively, the encounter date used by the enrollinprogram tag does not always use the appropriate date for the form.  It needs to consider the pending encounter date, the existing encounter date, the default encounter date, the current date, and any associated visit dates.
