Description
It would be quite beneficial to be able to choose the Encounter Type that an HTML form creates while the HTML form is being edited rather than when the form is being designed. In the HTML form design, you should be able to specify the Encounter Types that can be chosen for that form when it's filled out. For example, we wouldn't want a Pediatric Return Visit to be able to create an Adult Return Visit Encounter. Perhaps this could follow the same "order" syntax that is used for the encounter locations.

...James wants to create encounters where the type varies and is selected. But actually looking at my database now it appears that form.encounter_type is not required, so we could add this functionality to HTML Form Entry.
