Adding a PR template to openmrs-module-locationbasedaccess repository

The PRs that are created in LBAC repository are unclear if the user does not refer to the Pull Request Format from openmrs-core. Due to this, many of the PRs are not clear and causes confusion.

To avoid this I'd like to add a Pull Request Format, like there is one for openmrs-core, to the openmrs-module-locationbasedaccess repository.

The template proposed for openmrs-module-locationbasedaccess repository is same as that of openmrs-core and I hope it'll improve the standards of the upcoming PRs.
