Display program attribute value if present

Since OpenMRS core supports now program attributes, in the PIH Malawi implementation we capture now the transfer out location when the program is completed with a state of Transfer out:

This ticket is to slightly modify the core patientProgram portlet to display the program attribute value associated with a program.
