Concept Proposal Form is broken

The page where you resolve a proposed concept is broken, it might be related to the fact that some deprecated field and methods were removed in 2.0

Dev Notes:

• The form can be seen at /openmrs/admin/concepts/conceptProposal.form
• Looks like it fails when the names for the possible concepts are getting printed
• The concept selection field for the mapped concept seems to be broken too
