UI for managing mappings

As the system administrator, I would like to be able to install a module and go to the Manage Mappings page to configure that module to use metadata available in the system.

I'll describe the UI in words since "Add/Edit UI Mockup" doesn't work for me.

The Manage Mappings page should display 2 tables. One with not configured mappings (metadataUuid == null) the other below with all mappings.

In both tables show the following columns: name, description, source, code, referenced uuid, actions. In actions show Set and Retire links.

When clicking Set, show a new page with a table listing paged metadata of that type with name, description, uuid and actions columns. In actions show the Select link, which should display a confirmation dialog when clicked and take you back to the Manage Mappings page if confirmed. The table should be filterable as well by name and uuid.
