Disable create button throught the process of creating a new concept

When the user is adding a new concept to a dictionary of their choice, the create button is supposed to be disabled. This feature was implemented on this ticket OCLOMRS-340, however, at the moment, it's only disabled halfway throughout the process. This ticket is aimed to fix this issue.
