Description
Bug: If you have triggered an error message on a form (e.g. the Create Concept form), that message stays there the next time you open a new form. (This was addressed in https://issues.openmrs.org/browse/OCLOMRS-878, but this ticket is asking for a deeper fix.)
Work around/fix requested in this ticket: use a react hook to reset the form state when you recreate a form.
Acceptance Criteria:
- Trigger an error message when creating a concept (or on any form, e.g. Create Concept, Create Dictionary, or Create Source forms). (e.g. you can try to create a concept, dictionary, or source using an ID that already exists.)
- See error message. Leave that form.
- Open a new Create Concept form, scroll to the bottom.
- You should not see the error message from the previous form.
