Ability for Data assistants to see the number of forms entered

The DataEntryStatistics module allows admins to see what progress has been made by data enterers.

However, sometimes users want to see their status as well.

Add a feature to the DES module (http://svn.openmrs.org/openmrs-modules/dataentrystatistics/trunk/) that hooks to an extension point (https://wiki.openmrs.org/display/docs/Module+Extension+Points) on the "My Profile" page.
