Description

      If you create a custom HTML form with radio button style coded answers, when you view the confirmation screen before submitting the form, the concept number rather than the concept name of the coded answer appears for the first answer. If the second answer is chosen, it shows --.

      The code of the HTML form looks like this...

              <fieldset>
                  <legend>Yeux/Eyes</legend>

<lookup expression="fn.getConcept('CIEL:163309').name"/>

                      <obs conceptId="CIEL:163309" id="eye_exam" answerConceptIds="CIEL:1115,CIEL:1116" showCommentField="true" style="radio" answerSeparator=" " commentFieldLabel="

Notes:"/>

              </fieldset>
              <fieldset>
                  <legend>Oreilles/Ears</legend>

<lookup expression="fn.getConcept('CIEL:163337').name"/>

                      <obs conceptId="CIEL:163337" id="ears_exam" answerConceptIds="CIEL:1115,CIEL:1116" showCommentField="true" style="radio" answerSeparator=" " commentFieldLabel="

Notes:"/>

              </fieldset>
