Admin should be able to configure (through the UI) how forms should be displayed and edited

      Description

      The Manage Forms UI allows admins to add links for entry of forms, but it does not let you control how forms are displayed or edited.

      As part of that Manage Forms UI we should let the admin specify what "encounter display template" to use.

      The admin should also be allowed to specify the Edit behavior of the form. (The quick way to implement this is to implement new encounter display templates for "Not Editable" and for "Editable via HFE Simple Form UI")

      Scope

      The purpose of this ticket is to be a "quick-fix"; if RA-1247 has already been done, then this ticket needs to be rewritten.

      Dev Notes

      The built-in encounter display templates are here: https://github.com/openmrs/openmrs-module-coreapps/tree/master/omod/src/main/webapp/fragments/patientdashboard/encountertemplate (but generally the UI should dynamically allow you to pick from all available ones)

      Currently, the edit URL and form display behavior is determined based on the encounter type, but ideally we would change some underlying code so that this can be driven by the form.
