Enable the ability to add a sticky note to the patient dashboard

      Description

      Add a flag field on the patient header (so it is always visible) to allow the care provider to add a short note on the current patient.
      When clicking on this note, the field would become editable (something similar to what's done in JIRA itself when renaming the issue title for instance).
      This would use the usual 'obs' method, which means that track is being kept anytime this field is updated. When the user edits the field, the previous content observation still exists in the database but is only voided.

            To make all the display logic work fine, I've added the following CSS class to my directive style:

              .editable-hide {
                display: none !important;
              }

            No need for xeditable.min.css anymore.

            So here is my PR with the 2 latest changes PR #107

            Romain Buisson I have been looking at the Sticky Note placement, and I am wondering if its possible to move it to just after Contact Info rather than its own row below the names - it uses up a blank row with nothing else in it so ends up taking focus unlike sticky notes that you put on the side of your monitor not at the front .. Thoughts

            After conversation on IRC with Stephen Senkomago Musoke, we have agreed to leave it like that for now and wait for users feedback.
