Reference metadata module should install appointment roles only if the module is present

As part of this card, We have made appointment scheduling to be an `aware_of` dependency for `reference-metadata` module.

Right now, `reference-metadata` always tries to install certain roles that are for appointment-scheduling module. We should create appointment specific roles only when the appointment module is installed.
