encounterDiagnosesByObs should allow configuring checkboxes

encounterDiagnosisByObs should be extended to support additional attributes that will be forwards-compatible with encounterDiagnosis.

It should support marking a diagnosis as a "recurrence." This will map to the FHIR Condition clinicalStatus "recurrence." This can be modeled with CIEL:165908 and CIEL:1688.
