Add all patient encounters to context model

It should be possible to make forms & actions appear or not appear based on whether a patient has an encounter of a certain type at a certain time. For example, "show the annual check-up form if there is no annual check-up encounter in the last year." This will also work for simpler cases like "don't show the intake form if there is already an intake encounter."
