Remove patient allergy information

      As a doctor/caregiver
      I want to remove a patient's allergy
      So that the allergy list only show what is relevant to the patient at the time

      Note:
      Want to keep allergy in backend for audit reasons. However, deleted allergy should not be displayed on UI. It's no longer an allergy of the patient as far as doctors/caretakers and patients are concerned.
      All removal needs confirmation from the user

      Acceptance Criteria:
      -Each allergy listed on the main allergy screen should have a deactivate button next to it (see mockup)
      -Once clicked, confirmation screen pops up (can make like mockup or use default confirmation screen)
      -Confirmation screen needs to state if user is sure they want delete allergy from list of allergies patient has
      -If user clicks cancel, screen closes and user is taken back to main allergy screen. No changes are made
      -If user clicks yes, screen closes and user is taken back to main allergy screen. Allergy is longer a current allergy for the patient
      -Once an allergy is removed, it should not reflect in the active allergies list
