Edit Patient flow should allow for estimated birth date

Currently the Patient Edit flow is accessed from the header on the Patient Dashboard. This flow brings you into a flow to edit the patient demographics similar to the registration flow. This flow doesn't allow for entry of an estimated birth date.

This should work like the registration flow where Estimated Years and/or Estimated Months can be entered. (See screenshot)
