Registration App "Name" section should be driven by default name template

Currently, the registration workflow is hardwired to only ask for given name and family name (by hardwiring in usage of the nameTemplateGivenFamily name template).

We should change this so that the default name template, and it's line-by-line format, is used to determine which fields to display and in what order.
