      Description

      Based on our experience with our patient name searches in Haiti, we'd like to modify the existing patient name search as follows:

      1) Don't bind to the search to the front of the string (ie, do a name like '%queryString%', not name like 'queryString%'

      2) Reduce the number of matches required from 5 to 3

      3) Remove the UPPER function, as this causes a performance hit, and is generally redundant because the standard mysql collations we use are case-insensitive. For those that use a case-sensitive collation, a custom search can be written.

      4) Only search if search string length greater than 2 characters
