Encounter resource includes both voided and non-voided encounterProviders

The REST Encounter web service should return a list of only active encounter providers.
Steps to reproduce this problem:

• enter a clinic note and assign a provider to the encounter
• save the clinic note
• re-open the clinic note and change the provider
• save the clinic note
• the encounter websevice returns both encounter providers, the one that got voided and the active provider
