Add HL7 NTE (OBX comment) processing to ORUR01Handler

Comments for observations in HL7 messages come in the form of NTE segments immediately following the OBX. They have string length limitations so typically they are split into multiple NTE segments. We need to recognize these segments and add them as comments to the observations.
