Hidden Identifier Fields When Creating a New Patient

When creating a new patient, the fields are hidden for entering the identifier, and selecting the type and location. You must click the "Add Identifier" button for the fields to appear. It should display the fields without having to manually click a button. "At least one Patient Identifier is required for each patient," so let's eliminate the button click and show the empty fields for the first identifier by default. This behavior is exhibited in OpenMRS Version: 1.8.0 beta Build 17512.
