Add Allergy dialog box should show date picker widget for selecting the start date

    Description

      On the patient dashboard, go to the overview tab, and click the Add Allergy link, this will pop up a dialog box for allergy details.
      Clicking into the Start Date widget shows the date picker widget behind the dialog box and as a result, i cannot choose a date.
