Add New Concept Page Broken

The page to add a new concept or edit an existing one is broken in Version: 1.9.0 RC Build 26313.

The page doesn't finish loading, and there is no add/submit button after you fill out the form. Two basic concept info sections are displayed, one for each language on the system. If you click on French or English, it then only shows the one box.

FireBug says...

When editing a concept, the Mappings section isn't shown, and there are no add/submit buttons at the bottom of the form.
