Found multiple reference terms

It appears that the 2011121-1030k changeset may generate duplicate entries which then cause ConceptReferenceTermValidator to fail with:

"Found multiple reference terms with the code: J94.8 in concept source ICD-10-WHO"

For "J94.8" I see 14 entries. Some of them are identical and some have only different description. It revealed after upgrading the MVP dict and validating ConceptReferenceTerm with the validation module. I validated Concepts before, but ConceptReferenceTermValidator is not run with ConceptValidator.

We should fix the changeset to remove duplicates if possible.

To reproduce run the standalone with the MVP dict and go to Manage Reference Terms and try to save one.
