Description
If a Provider Attribute Type is retired, it should no longer appear when adding new or editing existing providers. It currently shows the retired field, and shouldn't.
Please ensure that when addressing this, we do not lose existing saved provider attribute values for that retired type, but merely don't show them.
See ProviderFormController for the method with @ModelAttribute whose value is providerAttributeTypes and make sure retired providerAttributeTypes that are excluded
