Add a location attribute to a Provider

When a provider logs in to the system there is no way to know where that provider is located. If a provider can be linked to a location (or locations) where they work, that could help in automatically choosing the location for activities like registering a patient. (If the provider works at multiple locations, this attribute can be an array of locations.)

Would like to see this in the Webservices.REST module as well.
