ORUR01Handler should load patient and concept from the datatabase

ORUR01Handler creates a concept and patient object by simply supplying the id in the constructor. This leads to an object that is not fully initialized leading to Null Pointer Exceptions for places that try access other properties of the domain object other than the id.
