Forms should not be required in HL7 ORU^R01 messages

The existing ORU^R01 processor requires a form to be in MSH-21, but Encounter does not require a form to be associated with it, and we want to be able to accept labs that do not have a set schema defined for them. This is easy enough to fix, by allowing getForm() to return null upon not finding uuid OR id.
