[Create Provider] Invalid validation for both empty Field and Both Filled Field

Problem :
1. The error message for the both fields can't be filled is displayed when both the fields are empty.
2. The Provider is allowed to create even when both fields are filled.

Steps to Reproduce:

1. Use the OpenMRS 1.11x version code and go the below link (with admin rights):
http://localhost:8080/openmrs/admin/provider/provider.form

2. Click on the Save Button without entering any data (see attached createprovider_1.png)

3. Enter the data in the both field, Person and the Provider Name
(see attached createprovider_2.png) and click on Save
