Drug orders for different formulations of the same concept should be allowed

Currently, the API doesn't allow placing multiple drug orders sharing the same generic concept even if the formulations are different. See the Order Uniqueness Constraints for discussion about order uniqueness. For typical drug orders, uniqueness is determined by the combination of concept + drug (i.e., concept_id + drug_inventory_id).
