Implementing Program Enrollment Attributes

Need a way to capture attributes for program enrollment. PatientProgram attributes will be implemented similar to patient attributes.

Dev Notes

Program attributes were implemented in Bahmni at aeb460, we need to move this work into core.

• The service layer and dao changes need to be moved to the ProgramWorkflowService and ProgramWorkflowDAO classes respectively.
• The changes in BahmniPatientProgram need to be moved to PatientProgram in core.
• The web services related changes need to be moved to the webservices.rest module.
