Add indices to order_number and accession_number columns in Order table

To speed performance when matching on order and accession number, we'd like to add indices on the order_number and accession_number fields in the Orders table.

Does anyone see issues with this? burke?

This involves adding the corresponding liquibase changesets in the liquibase-update-to-latest-2.4.x.xml file.
