Add not null constraint to encounter_diagnosis.dx_rank column

    Description

      Diagnosis.rank is marked as not nullable via the JPA annotations but encounter_diagnosis.dx_rank column seems to be missing the constraint in the database, seem like the constraint was implicitly dropped by the liquibase changeset in 2.5 where the column was renamed.

      Was this intentional or an oversight?
