Allow for death registration to be done via xforms

AFAIK, death registration is not coded into Xforms (i.e. person table, columns for dead, date_of_death, cause_of_death). This would be a nice feature to have! This could be hard-coded, or as from my previous post, it could just be done through a more generic table.column default xform parsing.
