Integrate UUIDs in xforms structure

For the mUzima Project, we need to use UUIDs instead of implementation site internal id in the xforms structure.

Please integrate UUIDs for:

• concept id
• form id
• encounter type id
• provider id
• patient id
• location id

It would be ideal to give implementers options to choose whether their xml code should include internal ids or UUIDs.
